ExitCare® Product Overview

ExitCare’s products are comprised of three categories, which can be integrated to provide the user with additional functionality and a unique capability:

  1. Core products that include enterprise-wide Discharge Instructions as well as other patient education materials
  2. Medication management products
  3. Added value optional modules

The medication management and optional modules can be integrated with the core ExitCare products to provide the user with additional functionality and a unique capability.

ExitCare Core Products

ExitCare can be licensed as an enterprise-wide solution or by specific care setting. The care settings include, but are not limited to support the following:Emergency Department,inpatient and ambulatory care settings. ExitCare is available for licensing to support ambulatory surgery centers and urgent care settings.

ExitCareED™ (Emergency Department)

ExitCareEDTM is a subset of the ExitCare Patient Information System and includes over 2,000 discharge instructions designed to meet the unique requirements of the Emergency Department. Most ED-focused documents have been reviewed by TSG (The Sullivan Group) to ensure the maximum risk protection for the user. ExitCareIP™ (Inpatient)

ExitCare Inpatient includesover 2,000 documents covering most medical specialties including Medical/Surgical, Cardiology, Ambulatory Surgery, Pediatrics, Obstetrics, Orthopedics, Sports Medicine, Dietary, Radiology, etc. Some titles are formatted to provide concise information at discharge, others are general informational documents. Where Joint Commission has specified Core Measures requirements for patient education, ExitCare documents will keep you in compliance. Details >

ExitCareAC™ (Ambulatory Care)

ExitCare Ambulatory Care features over 3,000 titles covering most medical specialties including Family Medicine, Pediatrics, Obstetrics, Orthopedics, Sports Medicine, Dietary, Radiology, etc. Some titles are formatted to provide concise information for an acute condition, others are general informational documents. ExitCare even has a full compliment of age-specific well child documents as well as a library of over 100 Diabetes-related documents. A perfect addition to any Electronic Medical Record system. ExitCare OnCall®

A patient portal for your hospital website with virtually no maintenance on your part - the content and functionality are delivered to your own website in an IFrame, and all documents are viewed or printed with your hospital logo. Details >

Languages

Languages that some ExitCare documents have been translated into include Spanish, Russian, Portuguese, Vietnamese, Tagalog, Bosnian, Korean, Haitian Creole, Canadian-French, Arabic and Traditional Chinese. New language translations are currently in development. The title list varies by language and new titles are translated on a regular basis.

Each of the core products include:

  1. Archive – Permanent archive of all printed instructions and medications prescribed provides proof of exactly what was given out to the patient for any future needs.
  2. Macros – Physicians can set up “macros” to combine topics, work/school release forms and prescriptions to automate the discharge process for common conditions.
  3. Patient Friendly Documents – The liberal use of graphics, bullets, subheadings and color all combine to encourage the patient to actually read their instructions.
  4. Extensive Library – You can provide your patient with clear and concise information on most medical conditions, helping to build a better relationship with your patient.
  5. Recent Patients List – This feature allows you to build a document set (and medication list) over time so you do not need to re-enter patient information and can see which topics have already been taught.

ExitCare Medication Management Products

ExitMeds®

ExitMeds® is the medication solution within ExitCare that is the gateway to the management of medications. There are several components to ExitMeds. They include:

  1. Electronic Prescription Writing – Reduces medication errors and speeds up the discharge process.
  2. Drug Information Sheets – Thousands of drug information sheets are available to print either as part of a prescription, or separately, in English & Spanish.
  3. Medication Reconciliation – You can clearly communicate to the patient the correct status of all medications - a Joint Commission requirement.

Thousands of defaults are pre-loaded into ExitCare so creating a script can literally take only a few seconds. You can also establish defaults by user, department or facility. Default meds can be set up by diagnosis, or bundled with documents as part of the “Macro System”.

Patient Medication Schedule allows the user the ability to create a patient friendly schedule, with dates, times and checkboxes of when to take medications. Multiple weeks can be printed at the touch of a button. This Module requires ExitMeds to be installed.

Drug to Drug & Allergy to Drug Interactions gives the provider the opportunity to conduct one final interaction check at the discharge event. If all medications have been entered to perform Medication Reconciliation, a simple touch of a button will provide a final quality check. The Interactions Module requires ExitMeds to be installed.

Rx Paper ExitCare has been specifically designed to meet the requirement to print prescriptions on security paper. The prescription is handled by ExitCare as a totally different print job from the rest of the patient instruction set, allowing the ability to specify a separate printer for scripts, or even a second paper tray within a printer. ExitLabs®

ExitLabs® contains over 300 patient friendly explanations of common lab tests. Where available they include normal ranges listed along with the ability to enter the patient’s results.

Patient Comprehension Questions

Patient Comprehension Questions allows for the objective documentation of patient comprehension. This can be especially important for low literacy patients as well as those patients who do not have English as their primary language. Over 600 topics have questions completed.

HL7 Interface

HL7 Interface offers unidirectional and bi-directional interfaces designed to import patient demographic data into ExitCare and export ExitCare patient record information out to hospital information systems.
