ExitCare Family Health Tracker Product OverviewFamily Health Tracker (FHT) is a Personal Health Record (PHR) that you can sponsor. It is a secure site to track family Medical Events and Medications. FHT also provides access to a large library of patient educational topics. You can evaluate FHT by visiting www.FamilyHealthTracker.com then click on “Start 30-Day Free Trial”. After the first 30 days, users pay $60 per year for the first individual profile and $30 per year for each additional profile. Each sponsoring organization will receive a unique “code”. When users register using the unique code, two things happen. First, the user will be “connected” to the sponsoring organization so when documents are viewed or printed, the organization’s “header” will appear on the documents establishing the brand for the organization. Second, the sponsoring organization will receive a portion of all revenue generated by users that register with the unique code. Today, users can manually enter data or grant guest access so a provider can enter data. ExitCare is working on several automated options to import data into the PHR. On the Overview Screen users can see a summary of the most important information in the profile, including Current Medications, Most Recent Medical Events, any events scheduled for today, the five most recent patient educational documents viewed, as well as the ability to print key reports. Users can switch to profiles of other members in their account to view or edit information. Every screen has a “Help” button which launches a 3 to 5 minute video clip explaining the components and features of the current screen. Library Screen
The Library Screen has the same powerful search features as previously described in ExitCare OnCall. Users can search for medical information by diagnosis or key word. They also have access to the complete library of over 2,400 First DataBank Drug Information Sheets. Many ExitCare documents are available in multiple languages and every document can be viewed and printed in a large text version. When users print or view a document, the document includes the sponsoring organization’s header. Medication Screen
The medication capabilities of FHT are the most sophisticated component of FHT. These features are a direct result of years of experience with ExitCare, our medical provider-based software. FHT checks current medications for interactions with the patients’ drug allergies and interactions between medications. Should a drug database update show a new interaction not previously known or significant news about one of the drugs listed in their profile, users automatically receive an email advising them to check their profile to review the new data or important news. Users can print out weekly medication schedules with dates, times and check boxes to easily monitor their compliance with their medication regimen. When users stop taking a specific medication, they enter the date of the last dose taken, then the medication moves to “Medication History” which can be reviewed at any time in the future. Finally, users can export the current medication list to give to providers should they be requested to provide a list of all current medications. This can be done from any computer in any location with internet access. Medical History Screen
The Medical History Screen allows the user to enter Medical Events using one of the twelve types of events (i.e. Doctor Office Visit, Hospital ED Visit, or Hospitalization). Users can also track Home Observations such as blood pressure readings, blood sugar levels, weight and temperature. They can even set up their own custom data type to meet their individual needs. Once data on any “metric” has been entered, the data can be printed or graphed (for example, a graph showing the systolic and diastolic blood pressure readings over a specific date range). Emergency & Guest Access Screen
Users can create both an Emergency Access card as well as provide caregivers with either read-only or read/write access to their profiles.
